The Geriatric Assessment is the heart and soul of developing a plan of care which solves some of the issues you are facing. A basic geriatric assessment will include the following components:
1. Physical Health
2. Mental Health
3. Functional
4. Economic & Legal
5. Social
6. Environmental
7. Spiritual
A thorough geriatric assessment can take several hours over a few days to compile the required information. An individual and/or family meeting is held to go through the assessment process. Family members and caregivers may be given a confidential questionnaire to complete which will be used to assess the client’s needs. Once all the information is gathered, we can start to build an individual plan of care that addresses the problems/issues which arose through the assessment process.
